What is MARPE?

MARPE (Miniscrew-Assisted Rapid Palatal Expansion) is an innovative orthodontic procedure designed to expand the upper jaw in older adolescents and adults, patients whose palatal sutures have fused and are no longer responsive to traditional Rapid Palatal Expansion (RPE).

Unlike standard expanders, MARPE uses a small, custom-fitted appliance anchored directly to the palate using mini-screws (temporary anchorage devices or TADs). This anchorage allows for greater expansion forces to be applied safely and effectively to the upper jaw bones.

  • We consider MARPE for patients up to approximately age 25.
  • MARPE is recommended only when clinically required, typically in cases such as:
    • A narrow palate
    • Crossbite related to transverse maxillary deficiency
    • Other situations where expansion is necessary for a stable orthodontic outcome

When we do not offer MARPE

  • We do not perform MARPE solely for the purpose of treating mouth-breathing.
  • Currently, there is insufficient scientific evidence to support MARPE as an effective standalone treatment for mouth-breathing or airway-related concerns.
  • If mouth-breathing is the primary issue, we may suggest appropriate assessment with medical professionals (e.g., ENT or respiratory specialists) to explore underlying causes.
When we do not offer MARPE

When is MARPE recommended?

MARPE is typically recommended for patients who:

  • Have a narrow upper jaw causing crowding or bite issues
  • Experience a posterior crossbite
  • Have airway concerns, such as snoring or mouth breathing
  • Are post-pubertal and no longer candidates for traditional RPE
  • Want to avoid surgical jaw expansion

By using mini-screws to engage the palatal bones directly, MARPE offers a powerful, yet conservative approach to correcting skeletal discrepancies in patients who have already finished most of their facial growth.

 

How does MARPE work?

The MARPE appliance includes a central expansion screw for gradual activation, four mini-screws placed in the palate to anchor the device to the bone, and a key that is turned daily to apply pressure and promote bone remodeling.

Over the course of 2–4 weeks, the mid-palatal suture begins to separate, allowing the upper jaw to widen. Once the desired expansion is achieved, the appliance remains in place for a few months to stabilize the results.

What to expect during MARPE treatment

Placement of the MARPE device is minimally invasive and is typically performed using topical or local anesthesia to ensure comfort. Patients may experience some temporary pressure or mild discomfort in the first few days following placement, but this usually subsides quickly.

As expansion begins, it’s common to notice a small gap forming between the front teeth. This is a normal and expected sign that the appliance is working effectively.

Throughout treatment, we’ll schedule regular follow-up visits to monitor progress and make any necessary adjustments. Many patients report not only improved bite function, but also enhanced facial balance and easier nasal breathing as a result of the expansion.

A consultation with our Specialist Orthodontist, Dr Neil Woodhouse, is £95, which we take on booking. During this appointment x-rays may be taken if necessary. On occasion a CBCT scan may also be required, which is not included in the consultation fee.
